Credentials and Application for Membership to the Main Line Health Medical Staff

A physician interested in becoming a member of the Main Line Health Medical Staff (Lankenau, Bryn Mawr, and/or Paoli campus/es) may request an application in writing. The request identifies the applicant’s specialty and campus/es where the physician will request privileges. The request is forwarded to the system office of Medical Staff Affairs to the attention of the President of Main Line Hospitals.

The application process includes at least:

Completion of a Pre-credentialing Form with required documents

Completion of a MLH Medical Staff Application and Privilege Forms with attachments (including, if appropriate, a log of procedures, explanation of claims, etc.)

Completion of clinical evaluations from current and past Department Chairmen

Interviews with the MLH Division Chief and Department Chairman in the applicant's specialty

Interview with the Credentials Committee
